Calendar/date picker for Appointment Form

This package might be useful for the DOB category on the appointment form: https://www.npmjs.com/package/react-calendar
I'm not sure if we also want to use that for the appointment date too or if we want to look into a different way to display available dates/times. Also, if we opt for just a calendar for appt times would we want a field on the form for preferred time: morning, afternoon, evening?
