Configure Database for different Practice types and Providers

The app is currently set up for Dental practices and gives no options as to the available providers in the office.

As the first means of expanding to different practices, there should be a practice type eg dental, medical, vision, and corresponding procedure types to suit (which will be reflected in the appointment form). For now:

• set up practice types in the Practice database
• set up available procedures
• set available providers for a given practice
