Background
Postnatal care is the care given to the mother and her newborn baby immediately after birth and for the first six weeks of life.

User story
As a village health worker, I should be able to access a postnatal form when I visit a girl that had a home birth delivery so that I am aware of the baby and the mother's health condition.
As a midwife, I should be able to access a postnatal form when a girl returns to the health facility after delivery so that I can report on the health of the baby and the mother.

Acceptance criteria

• The mobile application should enable the village health worker and midwife to capture the postnatal information of the young mother.

• Data should be validated.
• Data should be reflected on the dashboard.
• Health workers should receive a notification to make a postnatal follow up on the girls that have delivered.
