Add Command Palette commands for user-defined CPTs

Add Command Palette commands related to CPTs created through SCF:

• Listing instances.
• Adding a new instance.
• Editing an existing instance.

Partially addressed in #121; creating and listing are supported, but editing an existing instance is not.
