Fix readme structure / links

Read me now has a number of broken links. Also, for consistency, please rename `output_data` to `output`. My mental model is:

* `data`: raw data coming in. Not versioned.
* `build`: processed data partway through the pipeline. Not versioned.
* `output`: final outputs from the pipeline. Versioned.

Equivalent to how `make` and `make clean` work.
