ban cost rework #14
Description
Combinations ($&, $|) and $j extbans break the cost model, such as it is, for channel bans, and so it needs to be replaced/improved.
The current favourite candidate is a per-channel limit for non-$j leaf bans, however they're distributed, and a separate, smaller limit for $j.
