Error while Reporting a document uploaded

'Report' icon gives error

While reporting a document in a course after clicking on the 'Report' icon on the right side of the document, an error pops up.
The pop-up window says, " 'Error while reporting' Not Found ."
This prevents a student from reporting spam or duplicate documents already uploaded to the Archive. Kindly resolve this issue.
