Delete GAD/PHQ mistakes in registry

Need for Optimization
Currently can't delete a GAD or PHQ in the registry an entry error.
"Would be good to be able to delete a PHQ/GAD score that was entered by mistake"

Describe Desired Optimization
Add a delete button and have a confirmation when deleting. Not a true delete.

Describe Alternatives
Make it editable and have a way to acknowledge the entry was edited

Delete button not near the submit button. In future as this function to the app

Is this intended to be addressed in the app, in the registry, or both?

Is the solution a true delete, where no trace remains? Or should a social worker be able to see that an assessment was deleted?

The SW only asked for it in the registry but I will bring it up on Monday @ 11, both are great questions I didn't think about nor had they.
