Ability for SW to assign activity planning or tracking

Problem
SW would like the ability to assign activity planning or tracking from the registry and push the request to the app like with phq/gad requests

Describe Desired Solution
In the registry in the patients profile the SW can send a nudging patients to do or schedule activities from the intervention

This would be similar to the process and design of asking for a phq on the patient side except it would not be a scheduled message on the registry side
