Problem: There is some errant spacing in some comments. Fix the errant spacing.
Problem: Some trailing whitespace existed in a file. Remove the trailing whitespace.
Problem: Some trailing whitespace existed in the TUTORIAL file and README file. Remove the trailing whitespace.
Problem: _genders_copy_fill_node_index() has some unused variables. Remove them.
Problem: The utility function _genders_hash_copy() is not used. Remove it.
Problem: In genders 1.8 the decision was to allow empty files as a degenerate case of a genders file. Some left over code exists that had assumed a genders file without any nodes was an error. Remove all left over code that assumed an empty file was not allowed.
