PROBLEM
The CONTRIBUTING.md file provides a link to a PULL_REQUEST_TEMPLATE.md file but the file is missing.
SOLUTION
This PR adds the missing PULL_REQUEST_TEMPLATE.md file. This file is a duplicate of the same file in the root Pest repo.
